Client’s Bill of Rights and Responsibilities

Client’s Rights

  • You have the right to receive safe and high-quality therapy / services
  • You have a right to be shown care, consideration, and respect
  • You have a right to receive services in a manner which promotes your dignity, privacy and that promotes your autonomy and decision making
  • You have the right to have your individualized beliefs and cultural, spiritual, linguistic or familial needs/preferences respected. You have the right to receive interpreter services if required subject to funding availability
  • You have a right to receive services free from discrimination on any ground of discrimination prohibited by the Human Rights Code or the Canadian Charter of Rights and Freedoms
  • A patient who is First Nations, Métis or Inuk has the right to receive services in a culturally safe manner
  • You have a right to be informed about services, therapy, options and costs in a clear and open way, with information provided in a manner which is accessible for you
  • You have a right to be involved in planning and decisions regarding your therapy services
  • You have a right to designate a person to be with you during your therapy services, and/or to designate a person to be involved in planning and decisions regarding your therapy services
  • You understand that at any time you may refuse consent for further therapy services
  • You have a right to provide feedback or recommendations about your services without fear of discrimination or reprisal
  • You have a right to be informed of the laws, rules and policies affecting FunctionAbility, including Ontario Home and Community Care Services Patient Bill of Rights, and to be informed, in writing, of the procedures for initiating complaints about FunctionAbility
  • If receiving your therapy on FunctionAbility premises you expect the environment to be safe and smoke free
  • You expect confidentiality concerning details of your condition and therapy unless you have agreed to release of this information or unless there is a legal requirement to release this information

 

Client’s Responsibilities

Clients and their relatives and friends also have a responsibility to FunctionAbility. You need to:

  • Provide accurate information
  • Use equipment safely – in the manner in which you have practiced with / instructed by your therapist
  • Keep appointments, or provide greater than 24 hour notice of cancellations
  • Treat FunctionAbility staff with courtesy, respect and dignity
  • If you are receiving your therapy at home you must provide and maintain a safe environment in which you and your therapist can work.
  • Inform your therapist if you are receiving any other therapy services
  • Be involved and participate in the management and decision making related to your services, where appropriate
  • Pay for services promptly, when required
  • Provide honest and open feedback on the services received

Access to Information

If you require information about your care to be provided in an accessible format for your needs (examples could include but are not limited to large print, audio recording, braille), we ask you to speak directly to your treating therapist about this request.

Your Feedback About Your Treatment

Your feedback is appreciated by FunctionAbility. If you are dissatisfied with any aspect of your services, you can make a complaint to the Director of Clinical Services. Please reach out to info@function-ability.com or 905-764-2340 with your concerns, and your complaint will be directed accordingly.

All feedback, both positive and negative, is used by FunctionAbility to improve the services that are provided to you and to other clients.

This Clients Bill of Rights is in alignment with the Patient Bill of Rights as outlined in Ontario Regulation 187/22, under the Connecting Care Act, 2019.
