What is a day in the life of a FA Case Manager?

• Providing initial in-home assessments
• Develop and implement individualized treatment plans for patients experiencing a diverse range of neurological and orthopedic injuries and conditions;
• Help patients in establishing and meeting meaningful goals within their home and in community settings
• Liaise with a variety of community partners and rehab professionals to facilitate client goals and independence

What do you need to be successful?

• You are client centered and enjoy collaborating with a multidisciplinary team;
• You are a motivated self starter with superior technical, verbal and written communication skills;
• You have strong computer skills (i.e. ability to work in MS Word and Outlook)
• You have strong verbal/written communication skills and have the ability to work with different personalities;

Qualifications:

• You have a Master’s degree in Physiotherapy
• You are a member in good standing with the College of Physiotherapists of Ontario (including PT Residents)
• You have a valid Drivers Licence and access to a vehicle
• MVA experience is considered an asset
• Strong knowledge and competence with a variety of clinical treatment modalities

Compensation: $55.00 – $70.00 per billable hour
